Superior HealthPlan, a managed care organization in Texas and part of Centene Corporation, has opened its annual grant program for applications. The initiative aims to address non-medical factors affecting health, known as social determinants of health, by supporting community-based organizations serving low-income neighborhoods across Texas.

Since the launch of the grant program in 2018, Superior HealthPlan has distributed over $1.3 million to projects that work on removing barriers to health and wellness. This year, the company will award $10,000 grants each to ten organizations for a total of $100,000. The grants are intended for programs focusing on housing stability and support for Medicaid recipients, food security and nutrition for vulnerable groups, and healthcare access and literacy through education and support services.
Eligible applicants must serve the Medicaid population in Texas, not have received a grant or sponsorship from Superior HealthPlan within the past year, be based in Texas with services directed at Texas residents, participate in an end-of-term follow-up visit by December 15 to demonstrate outcomes from the funding, and submit required documentation by April 30. Applications must be submitted by March 31, 2026 through the Superior HealthPlan grants webpage.
Superior HealthPlan has operated in Texas for more than 25 years and currently serves about two million people statewide with approximately 3,600 employees covering all counties. Since 2020, the organization has contributed $12.3 million through various forms of financial support aimed at assisting low-income communities.
